What is the Membership Program?

For over a decade, Circle of Life Family Medicine has provided our patients with a higher level of care — same-day appointments, extended visits, 24/7 access to Dr. Pichay, and proactive health management. Our new Membership Program is how we continue offering that same level of care going forward.

This is not a change to your insurance coverage or how your visits are billed. All covered primary care services continue to be billed through your insurance exactly as before. The membership covers the enhanced services that have always set us apart — the things insurance doesn't pay for.

Is Circle of Life a Direct Primary Care (DPC) practice?

No. Circle of Life Family Medicine is not a Direct Primary Care practice. We accept and bill insurance for all covered primary care services. The membership fee covers only the enhanced, non-covered services described below — it does not replace your insurance.

Will insurance still cover my visits?

Yes. Your insurance will continue to cover all the same primary care services it always has. Copays, deductibles, and insurance billing remain exactly the same. The membership fee is separate and covers only the enhanced services that go beyond standard insurance-covered care.

Family plans cover everyone in a single household. If you add family members after enrolling, your rate adjusts to the applicable tier automatically.

Why are you doing this now?

For years, we have gone above and beyond what insurance reimburses — providing same-day access, 24/7 availability, longer visits, and personalized preventive care as part of how we practice. These services have real costs that insurance does not cover. The membership program allows us to sustainably continue delivering the level of care our patients have come to rely on, without compromising the quality of that care.

How do I sign up?

You can enroll online through our patient portal. If you need help signing up, call our front desk and we will walk you through it. Enroll by June 1, 2026 to lock in your founding member rate.

Can I enroll my whole family?

Yes. Family plans are available for households of any size, with per-person rates that decrease as your household grows (see pricing table above). All members must be in the same household. If you add family members after initially enrolling, your monthly rate will automatically adjust to the appropriate tier.

How do I cancel if I need to?

You may cancel your membership at any time by sending a written cancellation request to our office via email. Cancellations must be submitted in writing (phone cancellations cannot be processed). As part of the cancellation process, you will be asked to acknowledge that you are opting out of the enhanced membership services. Standard insurance-covered primary care visits remain available to all patients regardless of membership status.

Do I have to be a member to continue receiving the care I’ve come to expect?

The enhanced services included in the membership — 24/7 access, guaranteed same-day acute slots, extended visits, proactive care coordination, and monthly sauna sessions — will be available exclusively to members going forward. These are the services that have defined our practice for over a decade.

Membership is not required to remain a patient at Circle of Life. Standard insurance-covered primary care visits are available to all patients. However, if you want to continue receiving the same level of enhanced care you have experienced here, membership is how that continues.
